Obtain a Copy of Your Case File

Medicare enrollees are entitled to a copy of their case file under the Freedom of Information Act and/or the Privacy Act of 1974. If you would like a copy of your case file, please send your written request to C2C at:

If you are requesting the case file, you must sign and date the request. If a representative is requesting the case file on your behalf, your representative must include the legal documentation that shows they are authorized to represent you and receive this information.

Representative-Specific Guidelines

I am a family member or representative of a deceased beneficiary or the beneficiary’s estate
Provide documentation confirming your legal authority to act on behalf of decedent.

I am an attorney representing a deceased beneficiary
Provide valid authorization signed by an authorized representative of the deceased and documentation authenticating the authority of the signatory on the release authorization to represent the decedent’s estate.

I have power of attorney (POA)
Provide a signed POA.

I am a legal guardian
Provide proof of guardianship.
